390 organizations to provide holistic, person-centered care to Medicare beneficiaries with dementia.

The Center for Medicare and Medicaid Innovation (CMMI) has released the names of the organizations that will participate in the 8-year Medicare alternative payment model, focused on beneficiaries living with dementia, Guiding an Improved Dementia Experience (GUIDE). The participating organizations, sixty of which are CAPC members, span most US states and include a wide variety of Part B providers, including hospitals, medical practices, home health and hospice agencies, and more. Established programs began delivering services on July 1, 2024, while new programs began their one-year pre-implementation period. This model requires participants to follow a standardized approach, provide 24/7 support to caregivers, provide up to 2 weeks of respite services, and link beneficiaries to social services based on screenings. Participants may also contract with other providers, called "Partner Organizations," to meet these requirements.
