The Center to Advance Palliative Care (CAPC) is conducting a comprehensive environmental scan to identify initiatives aimed at improving the care of Black patients living with serious illness, and their families. Your response will help us identify and disseminate promising practices. Please be part of the first scan of its kind on this topic.

We want to hear about initiatives in planning stages as well as those already implemented. We also ask that you share this questionnaire with relevant colleagues across your organization. The questionnaire should take less than 5 minutes to complete.
