Disrupting Complex and Serious Illness Care Toolkit
A growing number of organizations understand that the traditional health care system is not working for frail older adults and others with serious illness—and are doing something about it. These innovators can use CAPC courses and resources to train their staff in the impactful skills that prevent crises, optimize quality of life for patients and families, and ensure cost-effectiveness.
Age-Friendly Health Care Learning Pathway
Age-Friendly Health Systems can deliver high-quality care to older adults. Their evidence-based 4M framework (what matters, medication, mentation, mobility) reduces patient harm by clarifying goals, managing medications, addressing cognitive issues, and optimizing mobility by assessing and addressing pain. This Learning Pathway provides comprehensive education on these topics, resulting in a CAPC Designation to demonstrate age-friendly care competency.
Customizable CAPC Handout for Patients and Families
This new handout contains patient and family-focused, frequently asked questions about palliative care. You can customize this version of the handout to include contact information for your palliative care program and your organization's logo.
Front-Runners Announced for the Tipping Point Challenge
On September 18, 2023, CAPC announced the front-runners for the clinical training category of The John A. Hartford Foundation Tipping Point Challenge. Until the end of October, there is time for other organizations to catch up to the front-runners. Winners will be announced in February 2024.

Innovation Category of the Tipping Point Challenge Starting October 1
The innovation submission period for The John A. Hartford Foundation Tipping Point Challenge will open on October 1, 2024. Initiatives must be already implemented with proven outcomes that are making breakthrough change in the care of serious illness. All settings, disciplines, and specialties, anywhere in the US, should participate! This category is open to all.
