A study published in the Journal of Pain and Symptom Management evaluates the outcomes of PATHS (Post-Acute Transition to Home with Supportive Care), a nurse practitioner-led telehealth intervention designed for patients with advanced cancer who are discharged from the hospital and decline a hospice referral. The PATHs program delivers palliative care during the critical two-week post-discharge period, aiming to address gaps in support. The findings suggest that PATHS may reduce hospital readmissions and more by addressing a critical gap in post-discharge support, with enhanced access to home-based palliative care.
