Charlson Comorbidity Index
The Charlson Comorbidity Index was developed by Mary E. Charlson, MD and colleagues as a weighted index to predict risk of death within one year of hospitalization for patients with specific comorbid conditions including heart failure, dementia, cancer and more.
This resource can be used by palliative care programs to identify patients in need of their services.
