Overview

Many community-based hospices are expanding their programs and services to support patients and family caregivers earlier in the disease trajectory. These expanded services aim to keep patients living with a serious illness at home and improve their quality of life. Services may include palliative care, home-based primary care, transitions programs, and the Program for All-Inclusive Care for the Elderly (PACE). Expanding to offer new services requires changes across almost all aspects of the organization and teams including branding, team organization, triaging referrals, internal communication and culture, and new partnerships.

This webinar, hosted by Tom Gualtieri-Reed, MBA, features a panel of three CEOs from community-based organizations who have expanded their services to include palliative care. Through a facilitated panel discussion, presenters will answer questions about their programs, and share what has worked (and what hasn’t) for them.
