What You’ll Do:
* Work in collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment and follow-up for patients referred to palliative care.
* Care for patients in their home within a 60 mile radius of our Tigard, OR. office
* Provide regular visits to reassess the patient’s needs, concerns, and update the plan of care
* Initiate communication with the Primary Care physician, other agencies involved in patient care
* Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law.
* Order and interpret laboratory and other diagnostic tests within the scope of professional practice.
* Order treatments and durable medical equipment as indicated
Qualifications:
* Licensed as a Nurse Practitioner in the state of practice
* Minimum of Masters of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law
* Training/experience in Palliative Care, Gerontology, Geriatric Medicine and/or Chronic Care preferred
* Prior Home Health and/or Hospice experience as an RN or NP preferred
* Practice experience with Part B billing preferred
* Have current DEA license
* ACHPN – Certification in Hospice and Palliative Care Preferred
* Minimum of two years of varied work experience as a professional nurse
* Must possess current CPR Certification
* Demonstrated skill in nursing practices and clinical judgments
* Ability to make home visits and provide nursing care in the home environment
* Ability to cope with family emotional stress and tolerance of individual lifestyles
* Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
* Valid driver’s license and proof of insurance is required
