Our current team of Nurse Practitioners and Physician Assistants bring a wealth of knowledge in primary care/internal medicine/palliative/hospice/geriatric experience and have all contributed greatly to our continued success. They each have an interest in home-based primary care with the goal of caring for older adults with complex chronic conditions in their homes, group homes, and independent living. Your Role:

- Deliver medical care in the patient's home and/or via telehealth.
- Perform comprehensive health assessments and diagnose / treat complex illnesses.
- Accurately and thoroughly document your patient encounter and ensure accuracy.
- Analyze test data to diagnose, treat and arrange appropriate follow up for the patient to ensure continuity of care.
- Educate patients and families on how to manage their health and well-being.
- Effectively navigate difficult conversations related to end-of-life issues and goals of care.
- Communicate effectively with patients, family, the medical power of attorney, other medical providers and all individuals involved in the patients care.
- Provide collaborative physician oversight for up to 6 practice APPs.

Experience and Qualifications:

- Minimum of three years of experience as board certified physician (MD/DO) in Geriatrics, Palliative Care, Family Practice or Internal Medicine with a passion for working with the Geriatric population
- Experience in home-based care a plus but desire to learn is welcomed
- Completed an accredited MD/DO program with a national certification
- Valid DEA Certificate in good standing with all schedules for prescribing
- Must not have any history of disciplinary actions toward their medical license
- Must not have any history of controlled substance actions toward their medical license
- Must not currently be under investigation

Job status: Full time or Part time

Compensation Rate: Competitive (based on experience); Incentive/bonus program

Schedule: Flexible; Monday Friday (within hours of 8-5)
