Description

Primary Care Nurse Practitioner (APRN)
Location: Lexington, KY
Schedule: Monday-Friday 8am-5pm (1 weekday per week after-hours call; 9 weekends and 1 holiday per year)

About the role:

The APRN exemplifies and promotes advanced nursing practice and excellence in patient care. Provides direct patient care and evaluates patient outcomes. Sets and evaluates practice standards. Provides consultation to nurses. Maintains own professional knowledge and skills as an expert and role model in Bluegrass Care Navigators (BCN) advanced nursing practice. Works in close collaboration with BCN's physicians through established palliative symptom management protocols. Assists in providing a plan of care for the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of BCN. This position reports to the Senior Hospice Medical Director.

Requirements
Is a graduate of a Master’s Degree in a Nursing Program. APRN certification as Family or Adult Nurse Practitioner. In-depth, current knowledge in area of specialty and the ability to translate professional and organizational goals into effective practice.
Is currently licensed as an APRN through the State Board of Nursing.
At least three (3) years general nursing experience Is required. Clinical work experience in nursing facility preferred.
Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
Committed to the research process and application to practice.
Ability to organize resources appropriately.
Strong interpersonal skills, communication, collaboration, and conflict resolution.
Knowledge of systems and the change process.
Ability to manage multiple priorities simultaneously and effectively handles the emotional stress of the workload.
Complies with accepted professional standards and principles.
Understands BCN's philosophy, principles of death/dying.
Possesses and maintains current CPR certification.
Have satisfactory references from nursing school, previous (or current) employers, and/or professional peers.
Has excellent observation, good nursing/medical judgment and communi­cation skills. Ability to provide good written documentation in a timely manner.
Is self-directed with the ability to work with little supervision.
Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.
Must be a licensed driver with automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
