Description
As part of the Nancy Renyhart Dementia Education Program (NRDEP) team, you will conduct psychosocial assessments for individuals living with dementia and their caregivers. This role includes Care Navigation, initial GUIDE screenings, ongoing support, and assisting in the development of social services within the care plan.
ESSENTIAL JOB RESPONSIBILITIES:
Include the following. Other duties may be assigned.
Maintains effective communication with the NRDEP and GUIDE colleagues, Pathways, Business Development Department, referral sources and local first responder departments.
Carries out responsibilities consistent with the guidelines for Care Navigation within the GUIDE model.
Maintains confidentiality of person with dementia/family information in accordance with agency and
professional standards.
Facilitates Virtual Dementia Tour® trainings, as needed.
Provides dementia care consultations for individuals with dementia and/or their caregivers to assess and uncover any psychosocial needs and makes home visits, as needed.
QUALIFICATION REQUIREMENTS:

A Master’s Degree in Social Work plus minimum of (1) one year experience as a caseworker with an emphasis on counseling, crisis management, community agency referral and grief support, preferred.
Must have a Valid Florida drivers License, maintain current auto insurance coverage and have access to a reliable automobile.
Experience in developing collaborative relationships with referral sources and first responders.
Ability to function independently and to adjust work hours to meet with program participants and to provide outreach and educational programs to family caregivers, healthcare professionals and first responders.
Ability to write routine reports and correspondence.
