Outstanding opportunity to be part of an established Palliative Care In-Patient team at a 200-bed community-based hospital with a busy Level 1 Trauma Center and Designated Primary Stroke Hospital. Being in one of the fastest growing areas of town conveniently located at the interchange of the 101 and I-17, the hospital is expanding to add an additional 100 beds by 2026. The in-patient setting spans sprawling suburban growth to the north and to inner city patients to the south. This creates a diverse and rewarding Palliative Care service.
HonorHealth is a six-Hospital not-for-profit healthcare system with a designated Palliative Care team at each hospital. The Medical Director Palliative Care will provide on-site management and in-patient consultation for both Deer Valley and Sonoran Crossing Hospital, a small 70-bed hospital eight miles north of Deer Valley. The Medical Director will also be a member of the HonorHealth Network Palliative Care Leadership team, Deer Valley Physician Leadership team and core faculty for the HonorHealth Palliative Care Fellowship program.
The Deer Valley/Sonoran Crossing Palliative care team consists of Palliative Care Certified Nurse Practitioner, Palliative Care Certified Nurse, dedicated Palliative Care Social Worker, and Chaplain. This is a team model where each morning the team reviews the patient list which averages 20-25 patients and evaluates the daily needs of the patient and family to determine who is best suited to provide the consultative service on any given day.
