Key Responsibilities:
• Partner with the Medical Director and care team to proactively identify patients eligible for palliative care and build a dedicated patient panel.
• Deliver sensitive and effective inpatient palliative care consultation, supporting patients with serious illnesses and their families.
• Conduct thorough patient evaluations, identifying immediate and ongoing needs, and incorporating the perspectives of family members and caregivers.
• Work closely with multidisciplinary teams to manage symptoms, coordinate care, and provide holistic support to patients.
• Establish rapport with patients, define their care goals, and develop a personalized care plan prioritizing comfort and quality of life.
• Educate healthcare teams, patients, and families on palliative care services, available resources, and best practices.
• Offer guidance and emotional support to families, assisting in challenging decision-making processes with empathy.
• Manage and document patient data meticulously, ensuring compliance with medical and legal standards.
These activities will be accomplished while ensuring the utmost compassion and sensitivity towards the unique needs and situations of each patient in the palliative care process.

Required Qualifications:

• Board Certification or Eligibility in Hospice & Palliative Care (Fellowship Trained)
• Valid MD/DO License in Arizona, free of Medicare or Medicaid sanctions.
• Expertise in Complex Patient Care
• Skilled in managing complex medication regimens and providing guidance in advanced illness management.
• Exceptional Communication Skills
• Strong interpersonal skills, with the ability to provide compassionate counseling to patients and families.
