Description

Join one of the nation’s largest, most respected health systems and experience what it means to live well. Advocate Health is seeking a Hospice Physician to join our physician-led medical group as an Associate Medical Director and serve its patients with Aurora’s Home Hospice program in Wisconsin.

• Part-time effort (0.50fte)
• Hours 8am – 5pm
• Shared virtual call approximately 1:6
• Evaluate patients for qualifying terminal diagnoses and continued eligibility for hospice care.
• Complete certificates of terminal illness and review interdisciplinary care plans
• Provide evolving symptom management, medication stewardship, and controlled substance monitoring for terminally ill patients
• Serve as attending physician patients who prefer not to return to the office, managing acute medical concerns for these patients
• Management of acute outpatient concerns and complications of our home hospice Attend interdisciplinary group (IDG) meetings per Medicare requirements
• Address polypharmacy concerns and ensure appropriate deprescribing while referencing our formulary
• Facilitate transitions for patients needing General Inpatient (GIP) or respite care to appropriate facilities
• Communicate and collaborate with care team members, providing education on hospice philosophy and benefits
• EPIC EMR
• Be part of an integrated nationally recognized organization with physician-led medical group and embedded service line
• Leverage access to DAX Copilot, an AI-powered clinical documentation tool integrated with Epic, streamlining clinical workflows by generating draft patient notes for efficient review, editing, and signature
• Experience a diverse and inclusive environment ensuring safe and equitable care for all patients

Training and/or experience required
• Completion of an accredited Residency Program
• Completion of an accredited Hospice and Palliative Medicine Fellowship, Preferred
• BE/BC
