We’re looking for exceptional Registered Nurses to become part of our highly esteemed Palliative care team. Location: Visits across Orange County

Schedule: Full-Time

Compensation: $95,000- $115,000 annually, commensurate with experience

Position Summary:
We are seeking a compassionate and skilled Registered Nurse to join our Palliative Care Program. This role focuses on providing holistic, patient-centered care in various settings, including patients' homes, assisted living facilities, and skilled nursing facilities. You will play a vital role in delivering high-quality palliative care, supporting patients and their families during challenging times.

Key Responsibilities:

Creatively manage resources and workflows to triage patient needs and address clinical demands.
Coordinate with the interdisciplinary palliative care team and referring providers to implement effective care plans.
Facilitate connections with complementary therapies (e.g., PT/OT, massage, music therapy).
Assist with non-pharmacological interventions to alleviate symptoms and enhance psychosocial coping (e.g., meditation, Reiki, journaling).
Support patients and families in legacy work to aid end-of-life coping.
Ensure continuity of care and consistent team presence across rotations.
Engage in advanced care planning discussions and assist with completing advance directives.
Deliver nursing care in patient residences and document assessments within the RN scope of practice.
Act as a resource for patient care units, outpatient clinics, and staff regarding palliative care.
Provide education to staff, patients, and families on symptom management, coping strategies, and end-of-life care.

Qualifications:

Active and unencumbered RN license in California.
Minimum of 2 years of nursing experience.
Palliative Care experience a PLUS
Associate’s or Bachelor’s degree in Nursing.
Community-based nursing experience, including home visits, preferred.
