Position Overview:
We are seeking a highly skilled and empathetic Palliative Specialist (PA or NP) to join our team. In this role, you will provide palliative care to patients, focusing on symptom management, pain relief, and improving the quality of life for individuals with serious health conditions. This position offers a blend of telehealth and in-person patient care, with the opportunity to collaborate closely with a multidisciplinary team of healthcare professionals.
Key Responsibilities:
Provide comprehensive palliative care consultations to patients with chronic or serious illness.
Develop and implement individualized care plans focusing on symptom management, pain relief, and emotional support.
Collaborate with physicians, nurses, social workers, and other healthcare providers to ensure comprehensive, team-based care.
Educate patients and their families about illness progression, treatment options, and goals of care.
Monitor patient outcomes and adjust care plans as necessary.
Participate in interdisciplinary team meetings and case reviews.
