Connie Hwang, MD, MPH
Connie Hwang, MD, MPH is a physician leader with expertise in population health and quality improvement. Dr. Hwang is the associate chief medical officer at Twin Health, a global health care company focused on personalized, data-driven approaches to reverse diabetes and chronic metabolic diseases. She builds high-impact partnerships with employers, health systems, and health plans that leverage Twin Health's sensor devices and artificial intelligence coupled with digital twin technology to help patients with chronic metabolic conditions achieve normal clinical values and stop medications. Prior to Twin Health, Dr. Hwang was chief medical officer and director, clinical innovation at the Alliance of Community Health Plans (ACHP), collaborating with clinical leaders across the country on topics of health care transformation, value-based care, community partnerships, and health equity. Previously, she was vice president of quality at Evolent Health and led improvement initiatives for Medicare Advantage Star Ratings, CMS ACOs, state Medicaid quality programs, and NCQA HEDIS performance. Dr. Hwang was previously vice president at the National Quality Forum and helped launch the Measure Applications Partnership, a public-private collaboration convened to review measures for CMS’ performance-based payment programs.
In 2020, Dr. Hwang was appointed by the GAO to serve on the Patient-Centered Outcomes Research Institute (PCORI) Board of Governors, which represents collective expertise in clinical health sciences and advises PCORI on its national research priorities and dissemination and implementation efforts. Dr. Hwang is a former RWJ Clinical Scholar and MPH graduate from Johns Hopkins and completed her internal medicine residency at Thomas Jefferson University Hospital in Philadelphia. She earned her medical degree from Mount Sinai School of Medicine in New York.
