
Jennifer DiBiase, DSW, LCSW-R, APHSW-C
Jennifer DiBiase, DSW, LCSW-R, APHSW-C is the Advance Care Planning Program Coordinator at NYU Langone Health, where she coordinates patient and staff education, process monitoring and evaluation, and the program's research and quality improvement efforts system-wide. Throughout her career as a Palliative Care Social Worker and Manager, she has utilized a trauma-informed approach to direct clinical practice, clinical supervision, and coaching of interdisciplinary teams to support patients and families coping with serious life-limiting illness. She has over 15 years of mentoring, supervising, and teaching experience including directing Mount Sinai's Post-Master's Palliative Care Social Work Fellowship from 2018-2023. She is currently an Adjunct Assistant Professor at the Silberman School of Social Work at Hunter College, CUNY.
