Stephanie Chow, MD, MPH

Dr. Stephanie Chow is an Associate Professor in the Brookdale Department of Geriatrics and Palliative Medicine at Mount Sinai Hospital. She serves as Director of the Aging, Life Innovations, Goals & Needs (ALIGN) Program, an innovative “high risk” intensive ambulatory care model that delivers comprehensive multidisciplinary care to older frail adults living with serious illness and complex care needs. ALIGN specializes in co-managing several vulnerable patient groups, including patients requiring repeated emergency room evaluations and prolonged hospitalizations, deliberating surgery, and transitioning between discharge settings. Dr. Chow also serves as a member of the Brookdale Diversity, Equity and Inclusion Council and is a passionate advocate for promoting equity in the underserved patient and inter-professional health system communities.
