Tara A. Cleary, DNP, GNP-BC, CHPN

As Associate Director, Member Relations Dr. Cleary is responsible for leading the CAPC member engagement efforts, ensuring CAPC members are engaged in CAPC content. Specifically, she will oversee health system and health membership portfolios, working closely with enterprise-level decision makers at these organizations to ensure alignment of CAPC resources with their overarching, high-level goals, and initiatives.

Tara A. Cleary is a Gerontological Nurse Practitioner with clinical and leadership experience in the acute care, long term care, community, and academic settings. She earned a Doctor of Nursing Practice degree from New York University in Advanced Practice Nursing where she was honored as one of two recipients of the prestigious Brookdale Foundation Scholarship. She is board certified in Advanced Practice Geriatric Nursing, Hospice and Palliative Care Nursing and is an End-of-Life Nursing Education Consortium (ELNEC) trainer. She has presented nationally on topics ranging from geriatric best practices, symptom management for older adults, and integrating competencies of older adults into cancer care. Dr. Cleary has published resources through the Hartford Foundation of Geriatric Nursing on utilization of the Palliative Performance Scale V2 and co-authored a medical textbook chapter on Frailty assessment in Treatment & Management of Cancer in the Elderly.

Most recent leadership experience includes developing and implementing multi state clinical care models to service clinically and socially fragile older adults and their caregivers to remain healthy and supported at home.

Tara holds a DNP, MA, and BSN from NYU, College of Nursing. She resides in Raleigh, NC.
