Building Community Partnerships with Senior CommUnity Care

In this episode, we speak with Senior CommUnity Care (SCC), a rural PACE program (Program for All-inclusive Care for the Elderly) which serves approximately 250 participants, in two counties on the Western Slope of Colorado. As part of Volunteers of America, SCC allows participants to age in place by providing individualized care from an interdisciplinary perspective.
